Limitations of Core Decompression Treatment for AVN of HIP

Core Decompression is the first thing doctors advise after diagnosis of avascular necrosis of hip joint. And you are told that this is the only way to stop the progress of the disease. But with almost hundred precent chances of failure of the core decompression procedure. Why do Core decompression is almost a failure? To find the answer of this question, we need to know about the process of the disease, impact of core decompression. And certainly – what results a surgeon look for, after core decompression? 

What is the process and purpose of Core Decompression

Core Decompression is drilling of the neck of femur. This surgical procedure is planned to make sure the pressure with in the bone will reduce through this process of drilling. Once this pressure will reduce, blood supply might start to the head of the femur. And this blood supply can ensures the nourishment of the tissue deep in the bone. So that progress of the Avascular necrosis can be delayed. 

Is core decompression can “avoid” total hip replacement

No, even after core decompression, a patient will have to go for THR. This is just a process to delay the THR. It cannot avoid the THR. 

What are the limitations of core decompression

There are several points which make core decompression not a very proper solution of the problem. Here are few points, which we need to consider- 

  1. Long duration of bed rest after the surgery
  2. Rarely delays the Total Hip Replacement
  3. Doesn’t Stops Degeneration of the bone

Let’s discuss these all one by one in detail-

Long duration of bed rest after the surgery: 

This is one big issue with the Core Decompression, it just seems a very simple and easy process. But healing of the hip joint takes very much long time and you need to wait to walk again for 3 to 6 months. That depends on the speed of recovery.

Rarely Delays the total hip replacement

Every patient who goes for Core Decompression always thinks that this surgical method will ensure avoidance of Total Hip replacement. But it is not so. In last so many years, I hardly met 2-3% patients where progress of the Avascular necrosis was stopped through CDR. But in most of cases, condition worsened with in few months and patients were told to go for surgery, with immediate effect. 

Doesn’t Stops Degeneration of the bone

The main aim of this surgical procedure is to reduce the pressure by drilling the bone. Which cannot ensure anyone that blood supply will resume in time or not. And until unless there is no blood supply, how can degeneration stop? Therefore degeneration continues and things keep on worsening. 

Therefore, if you are going for core-decompression with a hope that it will help you to avoid total hip replacement than this is not a perfect procedure to do so.
