The first diagnosis of Avascular Necrosis goes by chance in most of the cases. Because pain of AVN mimics with many different conditions. Specially with Sciatica pain. And in many cases the confusion- is it sciatica or avn- delays the treatment and complications keeps on increasing.

These are entirely different conditions. But in maximum cases, these two accompany in patients. Specially in patients of AVN. Because it is hard to walk straight for the patients of AVN, therefore spine gets complicated in long run and axis of spine is disturbed. This leads to the spinal disc problem.
There are many common features between sciatica and AVN of hip joint which cause confusion while diagnosing the disease.
A patient limps in both- AVN of hip joint and Spinal disc issues. This is the reason in many cases, the first doubt is on most common condition and that is- spinal disc problem or compression on nerve roots of sciatic nerve.
A patient in his early 30s was feeling limping while walking. He visited a neurologist and was misdiagnosed as “peripheral neuropathy”. Neurologist prescribed him – Anabolic steroids. It further complicated the whole condition of AVN.
In case of avascular necrosis of hip joint, obviously pain will be in hip joint. But the tributaries of sciatic nerve also supply the same location and it causes confusion between these two conditions.
Your walking gait is complicated with both diseases. This is another cause behind misdiagnosis of the conditions.
Inspite of all these common features and similarities. There are several differences in both the conditions. These differences help a physician to make a proper diagnosis. And a proper diagnosis leads to a complete treatment of the condition.

Pain makes these one, pain differentiates AVN and Sciatica
Pain is common between both conditions but location of pain and intensity of pain is entirely different in both conditions. The table below is accurate about the differential diagnosis between sciatica and osteonecrosis of hip joint.
In Sciatica: Pain is neural, therefore it is always severe. Movement complicates the pain. Rest helps in reducing the pain.
In Avascular Necrosis: Pain is more muscular. Rest doesn’t help in reducing the pain. Specially, pain increases more in night.
Pain of sciatica: radiates in whole leg. From back to the toe.
Pain of AVN: Mostly this pain is confined to the hip joint, groin and radiates to the knee joint only. Pain of AVN cannot radiate below the knee joint (in most of the cases).
In case of osteonecrosis of hip joint the movements which involve the hip joint are mostly restricted. You cannot rotate your leg right and left. You cannot fold your from hip joint.
But when it is sciatica, rotation of leg wont be affected. Movements where you have a stretch on your sciatic nerve- will only complicate.
Gait is- how do you stand and walk.
In AVN of hip joint when you cannot open your hip joint properly. You cannot put your feet properly.
But opposite to this in sciatica, a person leans on a side to relax the compression on the sciatic nerve.
